In order to claim your tax refund, you will need to fill out the "Tax Refund Request Application". If you do not have access to a printer, we will have additional copies when you come into the office. You must come into the EMS office at 55 Dakota Meyer Drive Greensburg, KY 42743 in order to retrieve your tax refund.
