In order to claim your tax refund, you will need to fill out the "Tax Refund Request Application" below. If you do not have access to a printer, we will have additional copies available when you come into the office. You must come into the EMS office at 55 Dakota Meyer Drive Greensburg, KY 42743

in order to retrieve your return.

To inquire about your personal refund, please utilize the search box highlighted in tan below. All you need to do is type your last name in the box and it will be highlighted on the list for your convenience. Scroll through the list and look for the tan highlight to easily locate your refund information.

Note: The highlight will disappear after 60 seconds, so utilize the scroll bar on the right side

of the green spreadsheet to find it quickly.
